As Case Manager with our Interim Housing Program, you work directly with young people on helping them achieve short and longterm goals such as more stable housing and employment and on the development of life skills.
The Case Manager with our Pathways Transitional Living Program supports young people in developing and achieving their housing and educational goals. Pathways is a unique program offering both a congregate living environment and apartment-living for residents.
As a Substance Use Specialist with our Health Outreach Program, you will support clients who use substances by being knowledgeable about substances and their effects, encouraging practices that increase safety and reduce harm, and providing referrals and support into treatment.
As a Health Outreach Bus Outreach Professional with our Health Outreach Program, you will build rapport with and provide services to Health Outreach Bus visitors in neighborhoods across Chicago. A valid driver's license, evidence of insurability and possession of or willingness to obtain a CDL Class B driver's license within six months of employment required.
